Job Description:

In this role, you will be integral in coordinating patient care outcomes, optimizing patient satisfaction, and enhancing cost-effective outcomes. You will work closely within our multidisciplinary health teams to deliver seamless, patient-focused care across various settings. The right candidate will be pivotal in facilitating the transition of care for patients, empowering them with information, and ensuring their journey towards health is well-coordinated and aligned with clinical best practices. Collaborate with healthcare professionals to plan and manage patient care plans effectively. Assess patient needs, planning interventions, and reviewing treatment plans and progress. Facilitate necessary resources and support systems for patients and families. Document patient care activities and outcomes for consistency in treatment across all care settings. Continuously evaluate the effectiveness of care and adjust plans as needed to achieve optimal patient results. Strong interpersonal and communication skills Ability to assess clinical data and health records Knowledge of healthcare regulations and policies Critical thinking and problem-solving abilities Proficiency in healthcare IT systems Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) preferred Licensed Registered Nurse (RN) in state of practice
